Diagnostic Medical Sonography Program
Application Packet Checklist
Application Available During Application Period
□ Download copy of Diagnostic Medical Sonography Application Packet - application is only available one week prior to and during the application period, March 1-15. Please refer to application packet for submission instructions as applications are now submitted online not in person.
□ Information on Application: Verify that all information entered into the application is complete and precise. Check your personal and MSJC email daily. Communication will be sent to your MSJC email address unless you do not yet have one, then we will send communication to your personal email address.
□ TEAS Results: Submit a copy of your unofficial TEAS results with your application.
Contact ATI directly at www.atitesting.com to request a copy/transcript of your official TEAS report to be sent directly to the MSJC CC AH program.
□ Copy of Nursing and Allied Health Evaluation (PROGRAM NOTES): Print out a copy of the nursing and allied health evaluation email that you received stating eligibility to apply and a copy of Program Notes that state your GPA, general ed. completion, language, etc. This information can be printed from the link found in the evaluation email. Once you click on the link, look at your program notes for information breakdown.
If you did not submit a nursing and allied health evaluation request, you are unable to apply to either of the programs at this time.
□ Current Licenses and/or Certificate: Please upload a copy of any Licenses and/or Certificates that you may have listed under section 2 of the Multi-Criteria Selection Point System.
□ Diplomas or Degrees: Upload a copy with your application.
□ Copy of Diagnostic Medical Sonography Workshop Certificate: Provide Certificate of Attendance or online Program Review of the DMS POWERPOINT PRESENTATION
How to Submit Application and Documents
Applications may be submitted through either of these methods:
- Scanned and emailed to the nursingandah@msjc.edu email address.
- Please Note: Documents MUST be in PDF or JPEG format
- MUST include subject line: FA2025 DMS Application
OR
- Downloaded, printed and delivered to the Nursing and Allied Health Department during
regular business hours.
- Hours: Mondays - Thursdays 8:00AM – 5:00PM
