Employee Forms

The following forms are intended for employee and department use only. 

The NEOGOV eForms Portal is available for current employees to update information pertaining to:

  • Direct Deposit Request

  • Employee Information Update

  • Tax Form Updates – Federal W-4/CA State Tax Withholding DE4

  • Name Change Request (for legal or preferred name change)

  • Emergency Contact Update – Emergency Contact/Beneficiary Designation
