Common Application College Report

Students needing a college official to complete part of the Common App College Report must:

1. Fill out the Transfer Institution Request Form
2. Fully complete the Student Information section of the College Report 
3. Email both the completed request form and College Report to Transfercenter@msjc.edu. Include the following in the subject line:
   
   • Transfer Institution Request: Student Name, Student ID Number, Date

Processing time is 5-7 business days
