Testing Services
Students approved for testing accommodation through DSPS may use this form to schedule
exams. Requests must be submitted at least 7 days in advance and should be scheduled
for the same date and time as your class, unless otherwise approved by your instructor.
Be sure to allow enough time for extended testing accommodations.
You are responsible for notifying DSPS of any changes to your exam schedule. Late
or individual requests are only considered in special circumstances.
