Request for Evaluation of Transcript or Prerequisite

If you are a transfer student from another college and have sent your official transcripts to us, please complete these forms for us requesting we evaluate your transcript(s). These evaluations will indicate if you have completed any course prerequisites and/or evaluate your courses for graduation requirements from Mt. San Jacinto College.

The Request for Evaluation of Transcript and Request for Prerequisite Evaluation forms are available online. You will need the free Adobe Acrobat® Reader installed on your computer to open these documents. If you do not have the Reader you can download it by going to: http://www.adobe.com/products/acrobat/readstep.html

INSTRUCTIONS:                     

These forms can be filled out on your computer. Type directly into the fields provided. Forms submitted by e-mail will not be accepted, please print and mail or hand-carry to the address provided. After printing, please check that all information typed is present. Follow the link to get the Request for Evaluation of Transcript form and the Request for Prerequisite Evaluation form.
