New Student - Intake Forms 


To receive accommodations from DSPS you will need to:

STEP 1: Complete steps 1-7 of the enrollment process.
                 Click the following links to assist you in this process:
STEP 2: View the DSPS Student Online Orientation. CLICK HERE to view.

STEP 3: Schedule your
DSPS INTAKE counseling appointment.
               CLICK HERE for DSPS contact information.

Once the Above has been completed, you will then need to complete the following:

STEP 4: On the day of your INTAKE appointment, please bring your
               disability documentation along with the DSPS Intake Packet.


              DSPS asks that you please have the above 3 forms, with disability

              verification, at the time of your INTAKE appointment with DSPS.  

              If you feel you may need assistance filling out the forms or have any 

              questions, please arrive 15 minutes early to your appointment.

Below are options for providing disability documentation:

  • CLICK HERE for the DSPS Disability Verification Form. Print this form, complete the top section, have a medical doctor fill out, and then bring with you to your intake appointment. 
  • CLICK HERE for examples of qualified professionals who diagnose disabilities.
  • Testing results or evaluation from high school with IEP (Individual Education Plan)
  • 504 Plan
  • Current Audiogram 
  • DD214
  • Psychological Exam/Testing 
  • Physical Exam 
  • LD Testing 
  • History from another college 
  • Eye exam from Ophthalmologist 


Please arrive at least 15 minutes early for all appointments.

Intake appointments will be 45 minutes. 
All other scheduled appointments will be 30 minutes or less.
