New Student - Intake Forms

To receive accommodations from DSPS you will need to:
  1. Complete steps 1-7 of the enrollment process. 
  2. Call or come into our offices to schedule a counseling appointment.
  3. Bring documentation of your disability to your appointment. ​

Below are the 3 forms you MUST fill out, sign, print and bring with you to your intake appointment with a DSPS counselor.  If you need assistance filling out the forms or have questions, arrive 15 - 20 minutes early to your appointment and we will be more then happy to help you complete them.

  2. Student Rights & Responsibilities​ 
  3. Consent to Release Information

We will also need a copy of your disability verification. Your accommodations will be decided based on these form(s). 


Below are options for providing this documentation:

  • DSPS Disability Verification Form Print this form, complete the top section, have a medical doctor fill out, and then bring with you to your intake appointment.  
  • IEP (Individual Education Plan) - Must have testing results or evaluation included.
  • 504 Plan
  • Current Audiogram 
  • DD214
  • Psychological Exam/Testing 
  • Physical Exam 
  • LD Testing 
  • History from another college 
  • Eye exam from Ophthalmologist 


Please arrive at least 15 minutes early for your appointments. Intake appointments will be no longer than 30 minutes.
